First Step: Apply for an Initial Evaluation

Let us arrange a personalized healthcare experience for you. To be scheduled for an initial evaluation you must complete a comprehensive application through our New Patient Application page. The application will also include several health questionnaire forms, a detailed medical history, and relevant medical records. Once you submit your application, our staff will review your materials and let you know if additions are required.
In preparation for your initial evaluation, Dr. McCloskey will review this information in detail and prepare comprehensive notes. The initial evaluation is an in-person 2-hour appointment that serves multiple functions:
✔ Dr. McCloskey will review and interpret your health history and perform a 2-hour comprehensive evaluation including a physical examination and functional movement screen.
✔ As part of your comprehensive evaluation, you will be asked to complete Dr. McCloskey’s advanced wellness panel which includes a blood draw.
✔ A second appointment will be scheduled for a report of findings and

Dr. McCloskey’s private Functional Medicine practice at Arete Health is limited to a very small number of patients. It takes tremendous time and energy for any healthcare worker to manage a case to our standard of depth and specificity. The patients who typically do well working with Dr. McCloskey are highly motivated and dedicated to evaluating their case in great detail. Successful management of such cases requires appropriate testing, diagnosis, financial commitment, and realistic clinical expectations.
Due to the limitations of Dr. McCloskey’s schedule and growing demand to train healthcare professionals, he can only accept new patients who are willing to complete a satisfaction survey for our clinical Proof of Concept (PoC) and share their case as an academic case study in-house with our interns and participants of our Professional Development Program and Arete University (both models focus on teaching clinical practice models to healthcare professionals interested in a nutrition and lifestyle-first approach to help improve the lives of patients suffering from a wide array of chronic conditions. During initial case management, the physical examination, history, and past medical records are formatted into educational resources with personal information redacted for privacy and HIPPA compliance, with the exception of the first name. Over time, added case management information may also be added to the case study materials.
Depending on the current clinic schedule and Dr. McCloskey’s availability, once an application is approved it can take between 2 and 8 weeks to see Dr. McCloskey in person as a new patient.
